The California Medicaid program was notified in October 2024 that federal regulators had approved the request to include traditional healing practices and other supportive services among the array of services to help persons recovering from substance use. This is the first time that the Medi-Cal program will cover traditional health care practices that are deeply rooted in cultural practices and have been shown to improve health outcomes, particularly for individuals with SUDs.

Medi-Cal will provide coverage for two new categories of interventions. The first, provided by traditional healers, includes music therapy (i.e., traditional music, songs, dancing, and drumming) and spiritual interventions (i.e., ceremonies, rituals, and herbal remedies). The second, provided through natural helpers, includes navigational support, psychological skill building, self-management, and trauma support.
In the coming months, the state will consult with tribes and tribal partners to develop guidance. The effective date for implementation seems to be several months away.
Medi-Cal joins several other states, including Arizona (Arizona (AHCCCS));
Oregon (Oregon (OHP)), and New Mexico (at New Mexico (Turquoise Care)) iin making traditional healing practices more accessible to Native Americans through Medicaid. Studies have demonstrated that these culturally centered approaches can enhance engagement and recovery outcomes, making them an essential component of holistic care for Native communities.
