Hot of the press! In late November 2024, the Substance Abuse and Mental Health Services Administration released guidance on implementing its 2024 final rule that made some of the most significant updates to standards for the operation of opioid treatment programs and the use of medications to treat opioid use disorder in close to 20 years.

Among the more significant areas of the guidance are the changes in take-home methadone dose protocols that were initiated during the COVID-19 public health emergency and the use of telehealth in treatment of opioid use disorder.

Appendix A contains a handy crosswalk from the 2022 rule to the 2024 rules.

The guidance document should be useful to a range of stakeholders, including patients, non-OTP clinicians, emergency department personnel, justice-involved individuals and justice-related staff, peer support and recovery community members, community leaders, healthcare administrators, and state regulatory agencies.
